An applicant for examination for a license as a cosmetologist, hair designer, advanced esthetician, esthetician or nail technologist pursuant to NRS 644A.300 , 644A.315 , 644A.328 , 644A.330 or 644A.345 , respectively, must provide: 1. A completed application on a form furnished by the Board. 2. A photograph of the applicant which must: (a) Have been taken not more than 90 days before the date of the submission of the application; and (b) Show the front view of the full face of the applicant in color against a plain white or off-white background and be otherwise substantially equivalent to a photograph suitable for use in a passport issued by the United States Government. 3. The applicable fees. 4. One of the following documents as proof of the identity of the applicant: (a) A copy of a current passport issued to the applicant; (b) A copy of a current driverโ€™s license or identification card, including a picture of the applicant, issued to the applicant by a state agency; or (c) A copy of a valid, unexpired Permanent Resident Card issued by the United States Citizenship and Immigration Services of the Department of Homeland Security. 5. If the applicant provided his or her social security number on the application for examination for the license, a copy of the social security card of the applicant. 6. If the applicant provided an individual taxpayer identification number on the application for examination for the license, a copy of the document issued by the Internal Revenue Service to the applicant indicating the individual taxpayer identification number of the applicant.
