Complaint alleging violation.

1. A person may file with the Commissioner a complaint alleging that another person has violated a provision of the Nevada Covered Service Provider Regulations or chapter 645F of NRS or an order of the Commissioner. 2. A complaint filed pursuant to this section must: (a) Be in writing; (b) Be signed by the person filing the complaint or the authorized representative of the person filing the complaint; (c) Contain an address and a telephone number for the person filing the complaint or the authorized representative of the person filing the complaint; (d) Describe the nature of the alleged violation in as much detail as possible; (e) Include as exhibits copies of all documentation supporting the complaint; and (f) Include any other information or supporting materials required by the Commissioner.
