Duplicate permit.

1. The holder of an expedited service permit may apply to the Department for the issuance of a duplicate permit if: (a) The permit is lost, stolen or destroyed; or (b) The holder wishes to have a different photograph of himself or herself affixed to the permit or to change his or her address on the permit. 2. Application for a duplicate permit must be made in person and include the required fee for the issuance of the duplicate permit.
