Do-not-resuscitate identification: Contents.

Each do-not-resuscitate identification issued by the health authority must include, without limitation: 1. An identification number that is unique to the qualified patient to whom the identification is issued; 2. The name and date of birth of the patient; and 3. The name of the attending physician or attending advanced practice registered nurse of the patient. (Added to NRS by 1997, 288; A 2017, 1769)
