Rule 145-3-06. Procedure for additional deposits

111.15 (A) This rule amplifies section 3.04 of the combined plan document and member-directed plan document. (B) A member participating in the combined plan or member-directed plan who makes a payment of additional after-tax contributions to the member's miscellaneous contribution account shall remit the payment with a form provided by the public employees retirement system.
