Rule 3737-1-18. Certificate of coverage

119.03 (A) The board shall issue a certificate of coverage to any responsible person who has met the criteria of division (D) of section 3737.91 of the Revised Code and rule 3737-1-04 of the Administrative Code. (B) One master certificate of coverage will be issued for each responsible person making payment to the fund and a facility certificate of coverage will be issued for each assured facility site. (C) The certificate of coverage shall contain the following information: (1) The amount of coverage to which the responsible person is entitled from the fund; (2) The time period for which the certificate provides coverage; and (3) The number of tanks which are included in the coverage.
