Use of form mandatory
July 24, 2002 Senate Bill 4 - 124th General Assembly Beginning one hundred eighty days after rules adopted under section 3902.22 of the Revised Code take effect, no third-party payer shall fail to use the standard claim form prescribed in those rules.
