Notification of change of address

September 10, 2012 House Bill 487 - 129th General Assembly Each person and each government entity that applies for or holds a valid license, certification, or registration issued under section 5123.161 , 5123.19 , 5123.45 , or 5126.25 of the Revised Code shall notify the director of developmental disabilities of any change in the person's or government entity's address.
