KIM HOLLAND INSURANCE COMMISSIONER State of Oklahoma This mailing consists of 2 (two) notices Second notice: June 1, 2003 TO: All Insurers Licensed to Transact Business in Oklahoma FROM: Carroll Fisher, Insurance Commissioner SUBJECT: Appointment Renewals We will be sending a list of appointments due for renewal on December 31, 2003, as we do each year. The list will be generated and sent by November 15, 2003. In the past, we have allowed the company to edit the list by indicating any appointment not to be renewed and remitting a check for those to be renewed at $40.00 each. Change notice: Beginning with this year’s renewals due on December 31, 2003, editing will not be allowed. The amount of the check to be remitted is $40.00 for each appointment shown on the list. We must have an appointment termination request in our office by November 1, 2003, for any appointment not to be renewed on December 31, 2003. It is our intention to start sending the renewal lists electronically, either on a disc or by e-mail. This has yet to be determined. We are sending these notices to the last mailing address we have been given for appointment processing. If the address for appointment processing has changed, please advise OID.
