Denial of application

(a) An application for certification may be denied for one (1) or more of the following: (1) Failure to meet any of the standards in the Act or OAC 310:657; or (2) Failure to provide timely additional information required under OAC 310:657-21-2. (b) Within ten (10) days after denial, the Department shall send written notice to the applicant. The notice of denial shall include a statement of the deficiencies on which denial was based and notice of the opportunity for hearing.
