Disclosure requirements

(a) In materials provided to insurers, insureds and employees,
the Plan shall include a summary of its utilization review
procedures.
(b) The
Plan
shall
clearly
disclose
employee
rights
and
responsibilities relating to quality assurance and utilization
review.
(c) The Plan shall provide to employees a toll-free telephone
number for information about the Plan's utilization review
program.
