Deeming application process

(a) An individual who desires to apply for deemed status shall
apply on the Department form which shall include, but not be
limited to, the following:
(1) Name, complete home mailing address, and telephone number
of the applicant;
(2) Copies of credentials which provide evidence of meeting
any of the criteria specified in 310:664-3-4; and
(3) A non-refundable fee of eighty dollars ($80.00).
(b) The Department shall notify the individual of the decision to
approve or disapprove the application within ninety (90) days.
