Centralization of reports

(a) All clinical information pertaining to a patient's stay shall be centralized in the patient's record. (b) The original of all reports shall be filed in the medical record. (c) All reports or records shall be completed and filed within a OAC 310:667 OKLAHOMA STATE DEPARTMENT OF HEALTH 42 September 13, 2019 period consistent with good medical practice and not longer than thirty (30) days following discharge. (d) A report or record requiring a physician or licensed independent practitioner signature is not considered complete until signed by the physician or licensed independent practitioner.
