Documentation of successful completion of the orientation must be provided to the Department within 90 calendar days of the executive director's designation. Failure to pass the orientation may result in: (a) Denial, suspension, or revocation of an agency's certificate. (b) The attachment of one or more conditions to an agency's certificate. (3) This rule does not apply to a person who has acted as an executive director for at least three years at an agency certified in Oregon under these rules.
