OAR 419-440-0140. Foster Care Agencies: Documentation Required When a Foster Care Agency Changes a Placement

Within seven working days after a child in care is moved out of a proctor foster home and placed in a different proctor foster home, a foster care agency must record all of the following information in the case record: (1) The reason for the new proctor foster home; and (2) The name and address of the new proctor foster home.
