OAR 441-810-0080. Office Closure

When an agency permanently closes an office, it shall notify the Director, by submitting the appropriate notice through the Nationwide Multistate Licensing System, within five days from closure and provide the following information to the Director: (1) Disposition or location of records and clients' moneys. (2) Name and address of a contact person for future inquiries.
