OAR 808-004-0350. Procedure if Information on Statement of Claim is Incomplete

The agency may refuse to accept a claim if: (1) The Statement of Claim does not meet the requirements of OAR 808-004-0340; and (2) The claimant fails to provide the missing information in response to a written request from the agency for the information within 30 days.
