OAR 855-115-0115. Responsibilities: Duty to Report

Each Pharmacist must report to the board as required by OAR 855-104-0010. In addition, unless state or federal laws relating to confidentiality or the protection of health information prohibit disclosure, a Pharmacist must report to the board without undue delay, but within 1 business day of: (1) Confirmed significant drug loss; or (2) Any loss related to suspected drug theft of a controlled substance.
