On forms approved and provided by the Commission, all wastewater utilities shall submit: (1) A financial Result of Operations annual report for the immediately preceding year, by April 1. (2) A report of all affiliated interest, intercompany, and intracompany transactions which occurred during the period from January 1 through December 31 of the immediately preceding year, by June 1.
