RHODE ISLAND DEPARTMENT OF LABOR AND TRAINING RULE 13 TERMINATION OF ACCOUNTS A. No employer’s account with the Department of Employment and Training shall be terminated unless and until the employer has provided the following: (1) A completed Termination of Account form. (2) Any and all completed quarterly tax forms. B. Any employer who terminates a business shall provide a completed Form DET-TX-17, Quarterly Tax and Wage Report, to the Department within ten (10) days of the termination.
