A. Description: Document the registration and enrollment of students in continuing education courses. Information includes student’s name, Social Security number, sex, ethnic background, date of birth, highest education level attained, type of work, years of employment, permanent address, courses registered for and signature of student. B. Retention: Until no longer needed for reference; destroy.
