A. Description: Record of homebound instruction provided for students unable to attend school for medical reasons. Information includes reason for homebound, dates of service, teacher performing service, application for homebound instruction, physician authorization, pupil’s name, and social security number. B. Retention: 5 years after end of fiscal year to which the records pertain, then destroy.
