ARSD 20:44:16:02. Form for undertaking

An applicant for an electrical contractor's or Class B electrician's license may obtain the undertaking form required by SDCL 36-16-20 at the commission office. The applicant shall fill out the form listing the applicant's full name, the city, the state, the name of the contracting firm (if applicable), and the date. The applicant must sign the undertaking form in the presence of a notary public. The applicant must file the undertaking form and the required $50 deposit with the commission office prior to the issuance of the license.
