ARSD 20:51:05:15. Prescription required to dispense a drug

A pharmacist may dispense a drug only pursuant to a valid prescription issued by a licensed prescriber. The prescription must contain: (1) The full first and last legal name and address of the patient; (2) The drug name, strength, dosage form, quantity prescribed, and directions for use; (3) The name and address of the prescriber; (4) The prescriber’s name and the date of issuance; and (5) For a controlled drug, the prescriber's federal Drug Enforcement Administration registration number.
