The following services require prior authorization:
(1) Medically-monitored intensive inpatient programs;
(2) Clinically-managed low intensity residential treatment programs for pregnant women or women with dependent children;
(3) Intensive methamphetamine services; and
(4) Psychiatric residential treatment programs.
