Enrollment--Form

The executive director shall establish a participation agreement form that includes: (1) The name, address, social security number, and birthdate of the participant and the participant's beneficiary; (2) The name and address of the participant's employer; (3) The participant's selection of investment alternatives; and (4) Any other information necessary for the administration of the plan.
