Section 56-40-101 et. seq., each employee welfare benefit plan that purchases a contract of insurance which provides for indemnification of claims made by plan participants, except insurance contracts that indemnify such plans only in the event of a catastrophic loss, shall register annually with the Department of Commerce and Insurance, in such manner as described on the attached registration form.
