Requests For Hearings Challenging Substantiation(S) For Which A

REFERRAL TO THE ABUSE REGISTRY WAS NOT MADE.
(1)
A person who has received a notice of the type specified in Rule 0465-03-01-.07(4) may
request a hearing challenging the substantiation.
(2)
A person who elects to take no action, or who fails to respond timely to the notice, waives all
appeal rights. The person’s name will be placed on the SIRI database.
