The facility shall designate the classification of Pediatric Emergency Care Facility it will
maintain and the level of care it will provide and submit this information to the Department of
Health on the joint annual report. If multiple facilities operate under the same provider
number, each geographically distinct facility shall designate the level at which it provides
service and will be surveyed at that level.
