Meter Reading Sheets Or Cards

(1)
The meter reading sheets or cards shall show:
(a)
Customer’s name, address and service classification;
(b)
Identifying number and/or description of the meter(s);
(c)
Meter readings and dates;
(d)
Identification of an estimated bill.
