Meter Reading Sheets Or Cards

The meter reading sheets or cards shall show:
REGULATIONS FOR ELECTRIC COMPANIES
CHAPTER 1220-04-04
(a)
Customer’s name, address, and rate schedule;
(b)
Identifying number and/or description of the meter(s);
(c)
Meter readings;
(d)
If the reading has been estimated;
(e)
Any applicable multiplier or constant.
