Educational Logging

Each applicant for a license or certificate will be required to prepare an educational log.
(1)
The educational log shall provide the following information:
(a)
Date of course
(b)
Name of course
(c)
Content of course
(d)
Provider
(e)
Total classroom hours
(f)
Location of course
(2)
An applicant may be required to provide additional information on education if deemed
necessary by the Commission.
