Change Of Address And/Or Name

(1)
Before engaging in the practice of fitting or selling hearing instruments, each licensee shall
notify the Council in writing of the address of his primary business where he is to engage, or
intends to engage, in the fitting or selling of hearing instruments.
(2)
If any changes occur in his place of business, the licensee must notify the Council’s
Administrative Office in writing within thirty (30) days of such change and reference the
individual’s name, profession, and license number. Failure to give such notice of the business
address change shall be deemed just cause for disciplinary action by the Council.
(3)
Each person holding a license who has had a change of address shall file in writing with the
Council his current mailing address, giving both old and new address. Such requests should
be received in the Council’s Administration Office no later than thirty (30) days after such
change has occurred and reference the licensee’s name, profession, and license number.
(4)
Change of name - A Licensee registered with the Council shall notify the Council’s
Administrative Office in writing, within thirty (30) days of a name change. A copy of the official
document evidencing the name change shall be included. Such request must reference the
licensee’s profession and license number.
