Fees

Every prepaid limited health service organization subject to this chapter must pay to the department the following fees: (1) For filing an application for a certificate of authority: five hundred dollars ($500); (2) For filing an amendment to organization documents that require approval: fifty dollars ($50.00); (3) Each annual report: one hundred dollars ($100); and (4) For each renewal of certificate of authority each year: one hundred dollars ($100).
