Health Insurance Identification Cards
The Utah Insurance Department (Department) issues this Bulletin to provide guidance regarding
health insurance information cards to insurers offering health insurance, which includes health
benefit plans, dental insurance, and vision insurance. It has come to the attention of the
Department that not all insurers are following the required standards for identification cards. One
concern is that insurers are not including the phrase “State Regulated” on all of their fully insured
identification cards.
Utah Code Ann. §§ 31A-22-614.5(3) and 31A-22-636(4) direct the commissioner to work with
various organizations to develop uniform standards for the electronic exchange of health insurance
claims or clinical health records. To accomplish this, the Department worked closely with the Utah
Health Information Network (UHIN), whose Board of Directors includes the Utah Department of
Health, the Health Data Committee, health care provider groups, and insurers. UHIN and
stakeholders representing the Board of Directors’ organizations worked with national
organizations to develop the standards.
The Department has adopted UHIN's standards through Utah Admin. Code Rule R590-164,
Uniform Health Billing Rule. Standards regarding identification cards are adopted pursuant to
R590-164-5(5)(n), Health Identification Card Standard v1.2, available at https://uhin.org.
All insurers offering health insurance, including dental and vision insurance, are expected to
comply with Health Identification Card Standard v1.2 for all policies or certificates issued or
renewed on or after July 1, 2023.
