R623-8-12. Documenting Problems

Any documentation required in this rule shall include reporting any problems or irregularities, and, if applicable, shall include: (1) details of any observed issue or problem; (2) the date and time of when any issue or problem occurred; (3) any action taken in response to any issue or problem; and (4) any resolution to any issue or problem.
