R156-90-301. Qualifications for Registration

(1) Under Subsection 58-90-101(5)(a)(iii), each health care services platform registration under Title 58, Chapter 90, Health Care Services Platforms shall be associated with a single, specific website domain name. (2) To register a health care services platform, an owner or responsible manager of the health care services platform shall: (a) submit a complete application for registration in a form approved by the Division; and (b) provide satisfactory documentation of registration with the Division of Corporations and Commercial Code.
