R911-11-6. Requirements to Obtain a Blood Draw Permit

(1) An individual who seeks to obtain a blood draw permit shall submit to the bureau: (a) an application on a bureau provided form; (b) documentation showing successful completion of bureau approved venipuncture training that is completed no more than one year before submitting the application; and (c) applicable permit fees. (2) Approved venipuncture training includes the following: (a) successful completion and passing of a college or university course for credit, that includes training in blood withdrawal procedures; (b) successful completion of a training course that prepares individuals to function in routine clinical or emergency medical situations and includes training in blood withdrawal procedures; or (c) successful completion of training that consists of no less than three weeks in blood withdrawal procedures and is performed under the guidance of a licensed physician. (3) Permit holders must notify the bureau within 15 days of a change in name or mailing address. (4) Permits are valid for three calendar years and expire December 31st of the third calendar year after issuance.
