R911-2-800. Emergency Medical Service Dispatch Center Designation Applications

An emergency medical service dispatch center shall provide: (1) name of the organization and its principles; (2) name of the person or organization financially responsible for the service provided by the designee and documentation from that entity accepting responsibility; (3) if the applicant is privately owned, they shall submit certified copies of the document creating the entity; (4) a description of the geographical area of service; and (5) a demonstrated need for the service.
