R386-702-12. Information Necessary for Public Health Investigation and Surveillance

(1) Each reporting entity shall provide the department or local health department with any records or other material requested by the department and necessary to conduct a thorough investigation, including: (a) medical records; (b) additional laboratory testing results; (c) treatment and vaccination history; (d) clinical material; or (e) contact information for any case, suspect case, or person potentially exposed. (2) Any individual or entity shall grant the department or local health department on-site access to a facility when the department or local health department decides such access is critical to a public health investigation.
