R386-703-5. Time Requirements for Reports

(1) An individual or agency required to report a reportable injury listed in Section R386-703-3 shall submit the report within 60 days of the date of diagnosis or recognition of injury. (2) In the event of an unusual or excessive occurrence of a reportable injury, the individual or agency required to report shall immediately report by telephone to the local health officer or the department.
