Vermont Department of Banking, Insurance, Securities and Health Care Administration Division of Health Care Administration BULLETIN HCA-109 RE: Regulation 93-4: Uniform Claim Forms and Uniform Standards and Procedures for Processing Dental Coding This Bulletin clarifies Vermont Insurance Regulation 93-4. It is the Department’s position that Regulation 93-4 applies equally to health care providers and health insurers. Pursuant to the regulation’s requirement that the most current dental terminology ("CDT") be used, the Department requires that dental providers and health insurers offering dental benefits submit, process, or reimburse all dental claims using CDT-3 no later than January 1, 2001.
