Providers qualified to deliver private duty nursing under the medically intensive children's program must have the following: (1) An in-home services license with the state of Washington to provide private duty nursing; (2) A contract with the department of social and health services/developmental disabilities community services (DSHS/DDCS) to provide private duty nursing; and (3) A signed core provider agreement with the medicaid agency.
