WAC 246-810-090. Application process for hypnotherapists

An applicant for a hypnotherapist credential shall submit to the department: (1) An application on forms provided by the department under RCW 18.19.090 ; (2) Applicable fee(s) required under WAC 246-810-990 ; and (3) Any additional supporting documentation requested by the department.
