(1) Applications must be completed on department forms. (2) License application forms may be obtained from and submitted to: Department of Labor and Industries DOSH Services Division Post Office Box 44655 Olympia, WA 98504-4655. Note: Purchaser and blaster license applications may also be obtained from explosive dealers or department service locations.
