WAC 388-101D-0215. Documentation of the individual instruction and support plan

For each client the service provider must: (1) Develop and keep a written record of the individual instruction and support plan that includes the elements required in WAC 388-101-3470 ; (2) Include a section or page in the individual instruction and support plan that provides or references all applicable support or service information pertaining to the client; (3) Review and update the plan to reflect changes in the assessed needs as described in the individual support plan; (4) Sign and date the plan's documents; and (5) Document the client's agreement with the plan as well as the client's legal representative if applicable.
