WAC 388-107-1090. License valid and not transferable

(1) The enhanced services facility is required to renew the license each year. (2) The license remains valid unless: (a) The department takes enforcement action to suspend or revoke the license per law; (b) The facility voluntarily surrenders the license and closes the facility; (c) The facility relinquishes the license; or (d) The facility fails to pay the annual licensing fee. (3) The facility license is: (a) Not transferable; and (b) Valid only for the provider and address listed on the license.
