DVR does not pay for any of the following medical treatments other than as an exception to policy, as described in WAC 388-891A-0205 : (1) Maintenance of your general health or fitness; (2) Cosmetic procedures, such as facelifts, liposuction, and cellulite removal; (3) Maternity care; (4) Hysterectomies, elective abortions, sterilization, and contraceptive services as independent procedures; (5) Drugs not approved by the U.S. Food and Drug Administration for general use or by state law; (6) Life support systems, services, and hospice care; (7) Transgender services including surgery and medication management; (8) Homeopathic and herbalist services, Christian Science practitioners, or theological healers; (9) Treatment that is experimental, obsolete, investigational, or otherwise not established as effective medical treatment.
