Ch. 4, § 1. Application Process

(a) Parents shall file an application with the Division. The application shall contain at least the following information: (i) Applicant name, address and phone number; (ii) Name of child for whom service is being requested; (iii) Nature of the child's disability; (iv) Eligibility status regarding the home and community based service waiver under Medicaid; (v) Other respite services the child is receiving; (vi) Household composition; (vii) Parent(s) gross income. (b) The Division shall determine eligibility in a reasonable time period. (c) The Division shall notify applicant of approval or disapproval of the application in writing. (d) Eligibility shall be reviewed at least annually. (e) The Division shall protect the confidentiality of all personal information gathered for the purposes of establishing eligibility.
