Ch. 3, § 2. Application

An ambulance service shall apply for a permit by completing an application provided by the Division. The application shall contain at a minimum: (a) A description of each ambulance including type, make, model, year of manufacture, and motor or chassis number; (b) An inventory verification of the recommended supplies and equipment carried aboard the ambulance; (c) Motor vehicle license number or aircraft registration number; and (d) The name and address of the person(s) owning and operating the ambulance.
